The EIS contains certain data captured from the Forms 5500, Forms 5500C/R
(Annual Report/Return of Employee Benefit Plan) and related attachments that
were filed with the Internal Revenue Service for plan years 1982 through 1998.
Employee benefit plans are required to file a Form 5500 or Form 5500C/R each
year. Certain welfare benefit plans maintained by small employers do not need
to file a Form 5500. The fields captured from the Form 5500 each plan year vary
for years 1982-1991, however, all fields were captured beginning in 1992. The
fields captured from the Form 5500C/R each plan year vary for years 1982-1989,
however, all fields were captured beginning in 1990. Purpose:
The data base was created to facilitate PWBA's disclosure and enforcement
responsibilities under the Employee Retirement Income Security Act as well as
for research activities. The data base is used to locate images of the Form
5500 filings and to create facsimiles of the Form 5500C/R filings to disclose
information in the filings to plan participants and others. In addition,
information from these forms is used to determine whether the plans are
operating according to the law and may be used for a variety of research
projects. Agency Program: The information captured by the
form(s) is used to carry out the law as specified in the Employee Retirement
Income Security Act of 1974 and Internal Revenue Code sections 6039D, 6047(e),
6057(b), and 6058(a). Plans are required to provide this information. It is
used by the Department of Labor and the Internal Revenue Service to determine
whether plans are operating according to the law
