1-Exposure to equipment hazards Checklist
a. Is access to this area limited to staff trained in helicopter equipment hazards? [1910.183(o) - Materials, Handling, and Storage, Helicopters]

d. Do employees approach or leave a running helicopter in full view of the pilot and keep in a crouched position? [1910.183(p) - Materials, Handling, and Storage, Helicopters]
e. Do employees refrain from administering CPR to patients by kneeling on transport carts while the carts are under the helicopter blades? 2-Exposure to High Noise Levels Checklist
a. Is proper ear protection given to all employees working in or around high noise exposure areas? [1910.95(b)(1) - Occupational Noise Exposure]
b. Are feasible administrative and engineering controls being implemented whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level (TWA) of 85 dBA, (decibels measured on the A scale). If such controls fail to reduce sound levels within acceptable limits is personal protective equipment provided and used to reduce sound levels within the levels of Table G-16[1910.95(b)(1) - Occupational Noise Exposure]
c. Do employers ensure that hearing protectors are worn by employees? [1910.95(i)(2) - Occupational Noise Exposure]
d. Is a hearing conservation program administered and continued whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level (TWA) of 85 decibels? Including requirements for monitoring, audiometric testing, audiogram evaluation, hearing protection, training and education, and recordkeeping. [1910.95(c) - Occupational Noise Exposure]



3-Fueling Hazards Checklist
