FCC Amateur Radio Forms
ARRL Members can submit an NCVEC Form 605 to
ARRL/VEC to Modify their Amateur License or to Renew
their Amateur License (with filing fee where required):

FCC Licensing Forms
- NCVEC Form 605 -- used for new license applications and
upgrades, license renewals and address, name and systematic call sign changes.
This form cannot be submitted to FCC.
- ARRL VEC Form 605-C -- used for new club or military recreation station licenses, modifications or duplicate requests of a club or military recreation station license or NON-VANITY club renewals or VANITY club renewals with associated FCC Regulatory Fee. This form cannot be submitted to FCC.
- Form 610-A -- No longer used for reciprocal operating. See International Operating.
- Form 610-B -- No longer used for applying for a new club station license, or for renewing a club, military recreation or RACES station license.See NCVEC form 605 above.
- FCC Form 605 -- used for license renewals and address, name and systematic call sign changes. Used to obtain a new vanity call sign. There is a charge for this service. See the vanity call sign page for more information. Use this form for direct correspondence with the FCC. This is the only 605 Form accepted by FCC.
- Form 159 -- to be submitted with a vanity call sign fee remittance
- Form 160 -- CORES registration.
- Form 606 -- Taxpayer Identification Number (TIN) Registration Form (used to register filers to allow amateur license renewals or changes electronically using the FCC's Universal License System [ULS]).

Getting FCC Forms via Mail
The FCC Forms Distribution Center will accept orders at 800-418-3676.
The ARRL/VEC also makes available most FCC forms for the amateur
service. You can obtain these forms by writing to:

Getting FCC Forms Electronically
FCC forms are available from the FCC via the Internet as shown in the table below. They also are available by fax at 202-418-0177. Via fax, use the document number from the table below or request the document index.
NCVEC Form 605
NCVEC Form 605 is used by
VECs (and can not be sent to the FCC). ARRL
Members can submit license modifications and license renewals on NCVEC Form 605
