Supplemental Educational Services

This is a sample application to become a Supplemental Educational Services (SES) provider. Please be aware that SES applications vary from state to state, but this proposal from the state of Minnesota may give you an idea of what it takes to become an approved provider of supplemental services. When you write your proposal, show the reviewers how your organization will help improve the academic success of children in your area.

It is important to remember that the Center for Faith-Based and Community Initiatives is not involved in the review process. Our office does not see the applications and has no input in the approval process. All applications to become an approved provider of supplemental services are reviewed by state departments of education.

Supplemental Educational Services is not a grant program. Your local education agency (LEA) must use funds from its Title I, Part A allocation to fund the services provided to the eligible students.

Carol M. White Physical Education Program grant

This sample grant application is from a previous competition for the Carol M. White Physical Education Program grant.
