SUBJECT: Hazard Information Bulletin - Defective 16 Inch Lasko Electric Fan with Metal Base Plate

The Directorate of Technical Support issues Hazard Information Bulletins (HIBs) in accordance with OSHA Instruction CPL 2.65 to provide relevant information regarding unrecognized or misunderstood safety and health hazards, inadequacies of materials, devices, techniques, and engineering controls. HIBs are initiated based on information provided by the field staff, studies reports and concerns expressed by safety and health professionals, employers, and the public. Information is compiled based on a comprehensive evaluation of available facts and literature, and in coordination with appropriate parties. HIBs do not necessarily reflect OSHA policy.

The General Services Administration (GSA) has issued a "Safety Alert" concerning a 16 inch fan with metal "base" plate, NSN: 4140-00-851-9319, manufactured by Lasko Metal Products Incorporated of Fort Worth, Texas.

The "Safety Alert" states that the subject fan poses a safety hazard with the potential of causing electric shock. The problem is attributed to design deficiency which may result in the loss of insulation and resulting in electric shock. The particular problem manifested itself in the form of sparking between the metallic surface of the fan base and the metal furniture on which the fan was standing. The Alert states that Lasko also manufactures a 12 inch fan which presents no safety problem, as it is not equipped with a metal plate at its base.

Compliance and consultation personnel should be aware of the hazards of the defective 16 inch Lasko electric fan with metal base plate.
