PRINTING AND DOWNLOADING NON-HTML FILES:
Please note that these forms are not designed for printing on standard 8.5 x 11" paper. If you want to print on 8.5 x 11" paper, you may need to change your Acrobat® application settings to use "Shrink to Fit", and you may also need to make configuration changes for your specific printer. It is also recommended that you do not attempt to print these forms from your web browser -- you should download the PDF file and open it directly in Acrobat®.

Image of a two button mouse, with indication to click on the right button It is recommended that when accessing NON-HTML file links, such as Adobe PDF, that you "RIGHT Click" on the hypertext links, and save these files directly to your local disk. Attempting to view or print PDF files through your browser with a plug-in viewer, can result in various technical difficulties.

Please contact your local PC support group if you have any problems downloading, viewing, or printing these forms. If you do not have the ability to reproduce the original 8.5 x 14" forms, you may want to contact a local printer or publishing company to have the forms professionally reproduced for you -- simply provide the printer with a copy of the PDF file on disk.

The Microsoft Excel template is customizable and exportable to other formats, but the end user assumes full responsibility for making all changes, and for the results generated by any such changes.

NOTICE:
In the December 17, 2002 Federal register (67 FR 77165-77170) OSHA announced its decision to modify the OSHA Form 300 to include an occupational hearing loss column. You can access the current forms by using the link above which refers to Calendar Year (CY) 2004 and beyond. If you need copies of the forms used for CY 2002 and CY 2003, you can access those forms using the link below referring to those years.
