Make a Self-Disclosure

Voluntary self-disclosures for violations which have occurred in U.S. EPA Region 5 (Illinois, Indiana, Michigan, Minnesota, Ohio, and Wisconsin) should be sent or faxed to Region 5's Enforcement Team, whose contact information is given below.

For violations which have occurred outside of Region 5, consult EPA Headquarter's self-disclosure and regional contacts page to find out whom you should contact in another Region to self-disclose.

Before self-disclosing, you may wish to consult one or more of U.S. EPA's audit policies which deal with self-disclosures and incentives for compliance. Depending on where you live, there may be a state self-disclosure policy applicable to you.
