Welcome to Erie County NY Connects - your "one-stop" for information about Long Term Care for people who live in Erie County. Finding your way around the Long Term Care system can be challenging. Our goal is to help you identify the best available care options, whether that means staying at home with the right mix of services or finding a place where your needs, or those of your loved one, can be met.

Long Term Care includes a variety of health and other services used by people of any age who need help to function in their daily lives. Long term care contains a mix of medical and non-medical services that can be provided in various settings. This may include care provided by family members, early childhood intervention, home-delivered meals, adult day care, case management, assisted living, personal care services, nursing, physical therapy and nursing home care.
