Northern New Mexico RFCC

The Northern New Mexico (NNM) RFCC is a collaboration between the Rio Arriba Family Care Network, Inc. (RAFCN) and Health Centers of Northern New Mexico (HCNNM). RAFCN and HCNNM play a central role in providing for the health care needs and well-being of residents of Northern New Mexico. RAFCN is a coalition of stakeholders, including providers, faith communities, local and state government agencies, consumer groups, school organizations, and community based organizations. Since the late 1990's, one major role of RAFCN has been to serve as the Health and Human Services policy advisory board and planning council for Rio Arriba County. HCNNM is a federally-qualified community health center that has served as the safety-net medical and dental provider for more than 18,000 individuals residing in 12 communities across a 22,000 square mile region of north central New Mexico since 1973.
Key NNM RFCC services and programs include:
- Telemedicine Initiatives;
- Technical Assistance for local health care centers;
- Providing a Women's Health Information Toolbox;
- Promotore Program;
- Comprehensive medical and social service needs assessment & referral;
- Social marketing for women's health;
- School Based Health Centers; and
- Direct medical and dental care.
