Nuclear Regulatory Commission Washington, DC 20555 Subject: Industrial Valve Operation, Dresser Valve And Controls Division, Dresser Industries, Inc. 10 CFR Part 21 Notification, Dresser Investigation File No. 95-01 Reporting of a noncompliance to a Main Steam Safety Valve that could cause the valve not to perform its safety function. Gentlemen: The following noncompliance has been determined to be reportable within the requirements of 10 CFR Part 21. The noncompliance occurred in a Main Steam Safety Valve owned by Entergy Operations, Inc., Arkansas Nuclear One (ANO), Unit 1. The noncompliance Is the use of a part In a safety valve that was not manufactured to specification requirements. The part was not manufactured nor provided by Dresser Industrial Valve Operation. Noncompliance and Brief History A Dresser main steam safety valve from Entergy Operations, Inc, Arkansas Nuclear One (ANO), Unit 1, was refurbished at Wyle Laboratories, Huntsville, Alabama, In February 1995. This was a Type 3707R safety valve with serial number BM-02978. During the refurbishment, Dresser's service engineer found the following nonconformances: (1) A foreign lower adjusting ring pin was found In the safety valve. The ring pin was not of Dresser manufacture. And was not In conformance with design criteria. The tip of the pin was worn to the point of failure. (2) The seal wire had been cut and stuck back into the seal. And had been re-crimped with regular pliers. Adjusting ring pins are required to be sealed using special pliers that impresses a Dresser Identification Into the seal. Adjusting ring pins are essential to the safety function. if the adjusting ring pin had failed In service, the lower adjusting ring will most likely rotate downward until It stopped on the nozzle threads. At this low position, the safety valve will not attain rated lift and capacity. And the safety valve will not execute Its safety function. Dresser is concerned about other safety valves at Arkansas Nuclear One (ANO). If safety related parts are used that do not meet design criteria, then we question the ability of the valves to execute their safety f unction. Also, the as found position of the upper adjusting ring may have caused the blowdown of the valve be as high as 15%.
