NRC to Meet With Holy Redeemer Hospital Officials
to Discuss Apparent Violations

The Nuclear Regulatory Commission staff will meet with officials of Holy Redeemer Hospital and Medical Center on Thursday, October 28, to discuss an apparent violation of NRC requirements that contributed to a misadministration earlier this year.

The predecisional enforcement conference will begin at 9:30 a.m., in the NRC's Region I Office, 475 Allendale Road in King of Prussia. The meeting is open to the public for observation.

NRC inspectors conducted an inspection at the Meadowbrook, Pa., facility on September 17 to review a medical misadministration. The inspectors found that, on September 14, a patient was administered an unintended dose of 5 millicuries of iodine-131 in preparation for a total neck and body scan, rather than an intended 300 microcuries of iodine-123 as part of an evaluation for hyperthyroidism. The technologist discovered that the misadministration had occurred on September 15.

Based on that inspection the NRC determined there was one apparent violation of NRC requirements: failing to provide adequate supervision to assure that a technician followed the licensee's written quality management procedures led to the misadministration.

The decision to hold a predecisional enforcement conference does not mean that the NRC has determined a violation has occurred or that enforcement action will be taken. Rather, the purpose is to discuss apparent violations, their causes and safety significance; to provide the licensee with an opportunity to point out any errors that may have been made in the NRC inspection report; and to enable the licensee to outline its proposed corrective action.

No decision on the apparent violations will be made at this conference. That decision will be made by NRC officials at a later time.
