- Eligibility for meat products for export to Western Samoa has not been
determined at this time. For further information contact the FSIS
Technical Service Center at (402) 344-5000.
Documentation Requirements
- Obtain FSIS Form 9060-5.
- For poultry and poultry products, the following statement must appear on the
export certificate:
"These poultry and
poultry products have been sourced from Newcastle and avian influenza
disease free areas, at least 50 km (30 miles), from areas under quarantine
restriction for those diseases."
Other Requirements
- The export must originate from within one of the 50 States in the United
States.
- FSIS Form 9060-5 must be signed by an FSIS veterinarian.
