Documentation Requirements*
- Meat/Poultry
- Obtain FSIS Form 9060-5--Export Certificate of Wholesomeness.
- All export certificates accompanying product must be signed by an FSIS
veterinarian. The veterinary degree (DVM or equivalent) must be indicated after
the signature.
- Hormones and antibiotic statement - In addition to FSIS Form 9060-5, type the
following statement in the "remarks" section:
"Based on the results of a national random sampling program, it can be
assumed that the meat and/or poultry products listed are free from harmful levels
of hormones and antibiotic residues."
Plants Eligible to Export*
At this time, all USDA inspected facilities are eligible to export to Guyana.
