Name formats for officers

You will recall that in our recent All-Hands Meeting, Dr. Gerberding asked all officers to support the Commissioned Corps by identifying ourselves as Corps officers in all our communications. This office has received many inquiries regarding the proper manner in which to do this.

The Commissioned Corps has not issued formal approval of name formats for officers. As soon as approval is received, we'll be sure to distribute. However, in the interim, please use the formats noted below.

Letter:

Jan Smith, M.D., M.P.H.
Captain
U. S. Public Health Service

Memo:

Bill Smith, M.P.H.
LCDR, USPHS

Email:

Can be either abbreviated or spelled out rank with degrees.

Most publications: use the same as the Letter format above.

Your working title may be used after the information above if you wish to do so.
