Subject: EXPLOSION IN THE VICINITY OF SEVERAL NUCLEAR GAUGES
Discussion:
On 3/25/99, the licensee notified RIII that its next leak tests for 40
Texas Nuclear Model 5199 gauges each containing about 100 mCi of Cs-137
would be late because they were inaccessible due to an explosion that
occurred on 2/17/99. The level gauges were in precipitator hoppers with
the manually-operated shutters open when a boiler exploded. The explosion
resulted in debris (including asbestos) that has prevented the licensee
from accessing the gauges. The hoppers containing the gauges have no
visible damage. The gauges were not in a fire.
The licensee has arranged to have TN perform a radiological hazard
assessment (including leak tests) on 3/29/99. Once a safe perimeter is
established by the authorized firm, the licensee plans to train clean-up
personnel about any precautions necessary for radiation hazard protection
during site clean-up. The licensee agreed to contact RIII by telephone
to: (1) discuss the results of TN's assessment; and (2) communicate if
any other general licensed devices were involved with the event.
Regional Action:
Region III notified the Missouri Bureau of Radiological Health on
3/25/99. RIII will communicate with the licensee to determine: (1) the
results of TN's assessment of radiological hazards; and (2) if any other
general licensed devices were involved with the event.

Subject: UNIT 2 FUEL LOAD
Discussion:
On March 25, 1999, the licensee commenced loading fuel on LaSalle Unit 2.
The licensee shut down Unit 2 in September 1996 for a refueling outage
and maintained the unit shut down to address performance and material
condition deficiencies. Extensive modifications were completed that
improved overall plant material condition. Several initiatives to address
human performance concerns were also completed. Prior to loading the
fuel, the licensee completed several reviews to ensure that systems
personnel were ready to reload the Unit 2 reactor. The extensive reviews
included reviews by plant and corporate management.
NRC action: Resident Inspector follow-up.
