Subject: STEEL BEAM KNOCKED OFF GANTRY CRANE DURING STEAM GENERATOR
         MOVE AT MAINE YANKEE

Discussion:

On April 7, 2000, at approximately 6:30 p.m., a top beam on a gantry
crane was knocked off during the movement of one of the steam generators
that was being removed from the Maine Yankee containment building. The
steam generator had been successfully lifted with the polar crane and
lowered to a horizontal position through the equipment hatch opening.
Once horizontal, a gantry crane outside containment was connected to the
lower end of the steam generator and raised to allow a special
multi-wheel transporter to be positioned under the steam generator. The
steel top beam of the gantry crane is supported by two hydraulic gantry
towers on either side of the transporter. As the transporter was pulling
the steam generator out through the equipment hatch and proceeding under
the gantry crane, the transition region of the steam generator (the
larger diameter, upper portion) struck the connecting element on the
underside of the steel beam of the gantry crane. The large beam was
knocked off the towers with one end landing on the ground and the other
end landing on the steam generator. No personnel were injured. The
contamination-control coating placed on the exterior surface of the steam
generator was abraded in several areas, but health physics personnel
confirmed no radiological contamination was released and the abraded
areas were re-coated. The steam generator was moved to its temporary
storage area. Maine Yankee suspended further lifting activities until a
root cause evaluation was completed and corrective actions were
developed.

Regional Action:

NRC staff will review licensee corrective actions before the remaining
two steam generators are moved.
