Subject: TECHNICAL SPECIFICATION REQUIRED SHUTDOWN DUE TO SAFETY RELIEF
VALVE ACTUATION

Discussion:
At 9:19 p.m. on Friday February 23, 2001, shortly after operators began a
planned shutdown to repair a safety relief valve (SRV), the SRV
unexpectedly opened and remained open until reactor pressure decreased to
130 psig. Operators performed a rapid plant shutdown in accordance with
station procedures, implemented applicable emergency operating
procedures, and by early Saturday morning placed the unit in cold
shutdown. The licensee is continuing its investigation of the cause of
the unexpected SRV opening.
During replacement of the affected SRV, the licensee found that some of
the studs and nuts used in the flange to connect the SRV tailpipe to the
SRV had fallen off and some stud/nut combinations in the flange had
as-found torque values that were less than the specified torque value of
230 ft-lbs. There was no evidence that the flange joint failed while the
valve was open and the licensee concluded that the as-found flange
configuration was sufficient to maintain operability. The licensee
inspected the SRV tailpipe to SRV flange on all Unit-2 SRVs and found
some additional examples of stud/nut torque valves less than the
specified value. The licensee preliminarily determined that the torque
value was insufficient to prevent relaxation of the bolting pre-load,
increased the specified torque value to 300 ft-lbs, and re-tightened all
stud/nut combinations to the higher torque value.
Regional Action:
The resident staff continues to follow the licensee's actions in response
to this event.

Discussion:
Effective February 21, 2001, Mr. Mano Nazar assumed the responsibilities
of Oconee's Station Manager following the resignation of the former
Station Manager (Mr. Jeff Forbes) from Duke Energy Corporation.
Mr. Nazar will also continue to serve in his previous position as
Oconee's Engineering Manager until a selection is made for that position.
