How to get a replacement Food Worker Permit

If you have lost or misplaced your unexpired* Washington State food worker permit and you took your last class and test at one of our class locations in King County, please fill out the form in full to request a replacement permit.
- It is required to retake the class and test in full every time your permit expires.
- If your unexpired permit is found in our records, you will be instructed to show up at any of our current class locations 30 minutes in advance of the class start time, show photo ID and pay $10.00. A new permit will then be issued to you.
- If you did not get your permit through one of our locations but elsewhere in King County, you likely got trained and tested by a private trainer who is registered to teach through the Washington Restaurant Association Education Foundation (WRAEF.) If this is the case, you can call Jennifer Dixon, the WRAEF representative who maintains enrollment records of everyone who got their permit through a private trainer in King County, toll-free at 1-877-695-9733, ext. 135 or by email at jdixon@WRAhome.com
