- Anthrax - report immediately; submit culture
- Botulism (Foodborne, Infant and Wound) - report immediately; sample submission for botulism: foodborne - serum, stool, suspect foods; infant - stool; wound - culture, serum, debrided tissue or swab sample.
- Brucellosis - report within 2 days; submit subculture
- CD4+ (T4) lymphocyte counts <200 and/or percent <14% of total lymphocytes (age >13 years) – report monthly
- Chlamydia, - report within 2 days
- Cholera - report immediately; submit culture
- Cryptosporidiosis - report within 2 days
- Cyclosporiasis - report within 2 days; submit specimen
- Diphtheria - report within 2 days; submit culture
- Enterohemorrhagic (Shiga-like toxin producing) E. coli, incl. E. coli O157:H7 - report within 2 days; submit culture
- Gonorrhea - report within 2 days
- Hepatitis A (positive IgM) – report within 2 days
- Human Immunodeficiency Virus (HIV) - report within 2 days (Western Blot , P24 antigen or viral culture tests)
- HIV infection (HIV nucleic acid tests [RNA or DNA]) – report monthly
- Listeria (blood/CSF) - report within 2 days
- Measles (rubeola) - report immediately; submit serum
- Meningococcal disease (blood/CSF/other sterile sites) - report within 2 days; submit culture or sterile site specimen
- Pertussis - report within 2 days
- Plague - report immediately; submit culture or other appropriate clinical material
- Rabies - (Human & Animal) report immediately (pathology report only; submit specimen upon request only)
- Salmonellosis - report within 2 days; submit culture
- Shigellosis - report within 2 days; submit culture
- Smallpox - report immediately
- Syphilis - submit specimen within 2 days
- Tuberculosis - report within 2 days; submit culture; report antibiotic sensitivity results to WA DOH for 1st isolates
- Tularemia - submit specimen within 2 days; submit culture or other appropriate clinical material
- Disease of Suspected Bioterrorism Origin - report immediately
- Rare diseases of public health significance - report immediately
