This service can be provided by your private provider or Public Health Center or a Community Health Center in your area.
For patients: Download and print this form (PDF) and write in your information, then call Harborview Patient Access Center at 206-744-8845. They will register you over the phone. Please bring the completed form and your registration number with you when you visit the clinic.
General information about TB Includes TB Annual Reports in King County, and common questions about TB, BCG vaccine, and QuantiFERON (blood test).
