Contract compliance forms and other requirements

This information applies only to Agency Services, Medical Pro and Consultant Contracts, which are the usual types of contracts made by Public Health-Seattle & King County. Be aware that contracts with Governments and Public Works contracts have different guidelines.
- Personnel Inventory Report
Contracts $25,000 and over: requests a breakdown of agency staff by ethnicity, gender and job title for the purpose of determining compliance with King County Code Chapter 12.16, Discrimination and Affirmative Action in Employment by Contractors, Subcontractors and Vendors. If your agency is very large or has many locations, then include only staff of the contracted program. This form is valid for two years from the date of completion.
- Statement of Compliance
Contracts $25,000 and over: needed only if contracted services are being provided by union employees. If agency does not have union employees or employees from an employee referral agency, “N/A” should be indicated on the form and initialed by the person completing the form, and the form should be returned with the contract package. This form is valid for two years from the date of signing.
- Affidavit and Certificate of Compliance
Contracts $25,000 and over: affirms and certifies compliance with King County Code Chapter 12.16, Discrimination and Affirmative Action in Employment by Contractors, Subcontractors and Vendors. This form needs to be notarized and is valid for two years from the date of signing.
- King County Consultant Disclosure Form
Consultant contracts only: this form seeks information about recent (past 24 months) contractor associations with King County to prevent conflicts of interest in King County contracting.
- 504/ADA Questionnaire and Assurance of Compliance
All contracts: a self-survey to determine if your agency meets ADA requirements to be accessible to disabled persons in employment and providing services. You return only the last two pages which need to be signed by you and notarized. This form is valid for two years from the date of signing, barring changes to the agency that would invalidate it.
- Equal Benefits Compliance Worksheet and Declaration Form
Contracts $25,000 and over: a questionnaire designed to determine if a contractor provides the same benefits to domestic partners as are provided to spouses.
- Summary of Insurance Requirements for Contractors
All contracts: a one-page summary of required insurance coverage and documentation for contracting with King County. This summary is intended as an aid to contractors and their insurance agencies to enable them to more quickly provide required insurance documentation.
- Credentials Requirements
Contracts that include the provision of professional health services: lists the minimum credentialing standards and documentation required for all licensed health care professionals which you must keep on file.
- Form W-9
All contractors new to King County: Needed to issue checks based on IRS requirements. Provide address information and Taxpayer Identification Number and sign the certification |
