Here's how to get a clinic appointment!

To schedule a clinic appointments with your provider please call 1-800-329-8387.

You should make every effort to keep your appointments.  If you miss two consecutive scheduled clinic appointments in the same clinic without contacting the medical center to reschedule, you will be discharged from outpatient care in that clinic.  To reschedule, contact 1-800-329-8387 to request a cancellation or to obtain another appointment.

Please report to the outpatient clinic no earlier than 30 minutes before the time scheduled for your appointment, unless you are requested to come earlier for pre-appointment testing.  This will decrease your wait in the clinic.
