Following a report by the Institute of Medicine (IOM) estimating that up to 98,000 Americans die each year as a result of preventable medical errors, President Clinton directed the Quality Interagency Coordination Task Force (QuIC) to evaluate the reports recommendations and to respond with a strategy to identify prevalent threats to patient safety and reduce medical errors.
