Fuels and Fuel Additives Reporting Forms

The staff of the Transportation and Regional Programs Division (TRPD) who were located at 501 3rd Street, Washington D.C. moved to:

This new physical address is for visitors and overnight couriers (FedEx, UPS, etc.).  Note that the U.S. Mail address was unchanged, including mail stop codes.  The telephone area code stayed the same (202), but the exchange changed from 564 to 343.  Most staff kept the last four digits of their current phone numbers but some were assigned new numbers.


A variety of EPA Fuels Programs require submission of forms by manufacturers and importers of gasoline, diesel fuel (including biodiesel) and fuel additives.  Some forms are used for registration of companies, facilities and products while others are used for compliance reporting.

Fuels and Fuel Additives Registration (FFARs)
This page contains reporting forms and associated instructions for the registration of fuels and fuel additives.  These forms are required to be filled out and submitted by producers and importers of gasoline, diesel fuel (including biodiesel) and fuel additives prior to their production or import.

Reformulated Gasoline and Anti-dumping Reporting Forms
This page contains registration of companies and facilities and compliance reporting forms and associated instructions for the Reformulated Gasoline (RFG) and Anti-dumping program.  Producers and importers of reformulated gasoline, conventional gasoline, or applicable blendstocks must fill out and submit these forms to EPA.  Some of these forms also serve programs other than RFG and Anti-dumping. An online registration form has been set up to facilitate the registration process for all programs.

Tier 2 Gasoline Sulfur Reporting Forms
This page contains compliance reporting forms and associated instructions for the Gasoline Sulfur program.  These forms are required to be submitted by producers and importers of gasoline destined for sale in the United States.  Registration and batch report forms are required in addition to these forms. Registration forms can be found on the Fuels Programs Registration page. Batch report forms can be found on the Reformulated Gasoline and Anti-dumping page. These forms may be submitted via EPA's Central Data Exchange.

Diesel Fuel Reporting Forms
This page contains compliance reporting forms and associated instructions for the Motor Vehicle Diesel Fuel Sulfur program. These forms are required to be filled out and submitted by producers and importers of highway diesel fuel. Company and Facility registration forms for this program can be found on the Fuels Programs Registration page. These forms may be submitted via EPA's Central Data Exchange.

Renewable Fuel Standard Reporting Forms
The Renewable Fuel Standard program was signed by the Administrator on April 10th, 2007 and starts on September 1st, 2007. Companies not signed up under RFG and Antidumping, Tier 2 Gasoline Sulfur, or the Motor Vehicle Diesel Fuel Sulfur and required to register under the Renewable Fuel Standard program may register for this program using the Fuels Programs Registration page. This page contains compliance reporting forms and associated instructions for the Renewable Fuel Standard program. These forms may be submitted via EPA's Central Data Exchange.
