Background: The recently discovered Kaposi's sarcoma (KS) Herpes virus (KSHV/HHV-8) is besides KS, associated with other conditions like the rare body cavity lymphoma/primary effusion lymphoma (BCBL/PEL) and multicentric Castleman's disease (MCD). The association of the virus with other lymphomas has remained controversial. Furthermore, possible sequence variation of KSHV/HHV-8 and association with various pathological conditions in Tanzania is not known. Methods: DNA from KS (n = 83) and ML (n = 25) including Burkitts lymphoma, (BL) 6, diffuse large B cell lymphoma (DLBCL) 12, precursor-B lymphoblastic (B-LBL) 1, Hodgkins disease (HD) 5 and BCBL/PEL 1 from Tanzania and Sweden were subjected to standard PCR for the 233-bp fragment of the KS-330 Bam sequence (ORF 26) of KSHV/HHV-8 DNA. In addition, this DNA fragment from 8 ML, 2 classical KS (CKS) from Sweden, 1 iatrogenic KS (IKS) from Sweden, 3 endemic KS (EKS) and 4 AIDS-related KS (AKS) was sequenced. Results: All KS samples had demonstrable KSHV/HHV-8 DNA. This was also detected in 13/25 (52%) of ML (2/6 BL, 6/12 DLBCL, 1/1 B-LBL, 3/5 HD, 1/1 BCBL/PEL) from Tanzania (n = 24) and Sweden (n = 1). The lymphoma PCR bands, with the exception of BCBL were clearly weaker compared to these of KS biopsies. Sequencing of the ORF26 DNA showed a relatively high conservation with few non-synonymous mutations which could be segregated into three main subgroups (A, B, C). Swedish cases segregated into A subgroup and Tanzanian cases into B and C. A subgroup segregation according to disease was not conclusive but only suggestive i a few cases. Conclusions: The uniform association of KSHV/HHV-8 with KS strongly suggests a pathogenic role. The non-uniform association with African ML suggests a general high prevalence of KSHV/HHV-8 in the population. Distinct KSHV/HHV-8 subtypes may be restricted to certain geographical areas rather than to distinctive pathological conditions. Supported by SIDA/TANSWED
