OBJECTIVE: To describe Kaposi's sarcoma (KS) in African pediatric cases. METHODS: 4 cases of KS occurring in children have been documented. Case report No. 1. A 8 month-old male African infant was hospitalized for chronic mild diarrhea and disseminated lymphadenopathy. He also had fever, cough, weight loss, herpes-like stomatitis and hepato-splenomegaly. A dozen of black-violet or dark brown skin nodules were present. The presence of antibodies to HIV-GP110 was detected by Western blot. Pathological examination of a lymph node a skin nodule confirmed the diagnosis of KS. The lymph node showed lymphoid follicles with lage germinal center and mitotic activity and vascular hyperplasia. The pathological aspect are compatible with a KS at an early stage, with the special infiltrative aspect of AIDS-related KS. Histology of the skin showed vascular hyperplasia and proliferation of spindle cells, with large number of adjacent lymphocytes. The evolution of the disease was fatal within 2 months. The peculiar aspects of the other 3 cases will be described. CONCLUSION: This first pediatric case of KS was typical of the series of four cases who were referred to the pediatric ward in one year.
