OMHD funds the Asian American and Pacific
Islander Health Forum (APIAHF) to promote the health and well being of Asian
Americans and Pacific Islanders (AAPI) through develop capacity of
community-based organizations (CBOs) and coalitions to meet the health needs of
AAPIs communities by providing leadership in public and community health
practices, collaboration and partnerships. |

APIAHF has implemented culturally and linguistically appropriate health
education; promote ethnic media, social marketing and capacity building
assistance projects targeting AAPIs. |
|
|
|
|
|
APIAHF principal goals are: |
|
|

Increase health professionals who are versed and vested in AAPI health issues to
become leaders and strong advocates; |
|
|
|
Strengthen and improve the quality and level of expertise of AAPI health
knowledge through the Health Brain Trust; |
|
|
|
Empower the AAPI community through capacity building and technical assistance. |
|
|
|
|
|
The purpose of the program announcement is to: |
|
|
It is anticipated that the project will produce leaders in community-based
public health that will have the ability to advice, design, implement and
evaluate AAPI policy analysis and health education to eliminate health
disparities in HIV/AIDS, cancer, diabetes among AAPIs through community capacity
building and leadership development in AAPI communities. Program outcomes
represent measurable advances in CDC’s Health Protection Goals and Healthy
People 2010. |

|
|
APIAHF
is responsible for the following activities: |
|
|
|
Increase the number of individuals who have the education, skills, and
experience to become leaders in the movement for AAPI health; |
|
|
|
Develop one major policy paper per year with the involvement of advocates for
AAPI health in a “Health Brain Trust”; |
|
|
|
Increase the capacity of community-based organizations and coalitions; |
|
|
|
Provide technical assistance and training on the use of demographic and
epidemiological data on AAPIs; |
|
|
|
Train members of the ethnic media to report on AAPI health issues and train AAPI
community health advocates to utilize media tools and strategies. |
|
|
|
|
|
Evaluation Measures: |
|
|
|
20 in-depth key informant interviews with eight HAPPI fellows to evaluate the
accomplishment of the program; |
|
|
|
An annual qualitative survey will be conducted with the members of the Health
Brain Trust to monitor the progress and effectiveness of the activity; |
|
|
|
Pre- and post-tests for each training/workshop will be conducted for each of the
four components; |
|
|
|
User’s survey will be conducted and registered APIAHF website users to provide
feedback, health data/cultural competency materials posted online, and resource
available; |
|
|
|
Progress and success of the program will be determined by the overall reach of
its activities to AAPI health advocates, CBOs, and community members; |
|
|
|
Evaluation consultant will assist in developing evaluation tools, analyzing
qualitative data, and providing recommendations for program efficacy and
enhancement; |
|
|
|
The evaluation plan will include qualitative and quantitative techniques and
will be participatory and dynamic in nature. |
|
|

Association of Asian Pacific Community Health
Organizations (AAPCHO)
"Strengthening Existing National Organizations Serving Racial and Ethnic
Populations Capacity Development Programs: Strategies to Advance Program
Implementation, Coordination, Management, and Evaluation Efforts." |

OMHD funds the Association of Asian Pacific Community Health Organization (AAPCHO)
to build organizational capacity to increase access to quality culturally
appropriate health care for underserved Asian Americans and Pacific Islanders (AAPIs). |
|
|
AAPCHO and its 23 community-based member organizations (CHC) located in
communities with high concentrations of medically underserved Asian Americans
and Pacific Islanders have developed, coordinated and implemented at least 6
national/regional health education prevention and promotion programs that
address risk factors and disease prevention for AAPIs in at least 5 AAPI health
disparity areas (diabetes, cancer, HIV/AIDS, tuberculosis, hepatitis B). Each
program emphasizes training, technical assistance, and the development of
resources for CHCs and other organizations. |

|
|
|
AAPCHO principal goals are: |
|
|
|
Improve the health of underserved AAPIs by increasing national and member AAPI-serving
organizations’ capacity to provide culturally and linguistically appropriate
public health care services through partnership building, health advocacy,
promotion, education, and prevention; |
|
|
|
Facilitate collaborative efforts and partnerships among private and public
health organizations, professionals, academic institutions; |
|
|
|
Increase awareness of AAPI health issues among policymakers and health
advocates; |
|
|
|
Provide technical assistance and training to CHCs to deliver culturally
appropriate health services to underserved AAPIs; |
|
|
|
Increase the availability and accessibility of health education for AAPIs. |
|
|
|
|
|
The purpose of the program announcement is to: |
|
|
|
Enhance existing community health services and serve as a catalyst for new
opportunities; |
|
|
|
Strengthen CHC organizational infrastructure and increase providers,
organizational, and community capacity to serve underserved AAPIs; |
|
|
|
Develop community action plans to sustain the CHC project sites at the
conclusion of the project. |
|
|
|
|
|
Evaluation Measures: |
|
|
|
Evaluation consultant will develop a logic model to guide the evaluation of this
project in collaboration with project staff and the project sites; |
|
|
|
Will provide an overall framework on how the program is intended to work; |
|
|
|
Will provide a strong foundation to conduct the ongoing evaluation to identify
the measurable outcomes that are benchmarks along the way to achieve the
long-term goal of improving the health status and access of AAPIs; |
|
|
|
AAPCHO will measure each set of events to determine the effectiveness of each
activity; |
|
|
|
Will employ a participatory and collaborative evaluation approaches; |
|
|
|
Will integrate components of the 6-step CDC Framework for Evaluation. |

|
|
|
|
AAPCHO
is responsible for the following activities: |
|
|
|
Identify new partners to collaborate and exchange public health information
through publication; |
|
|
|
Promote AAPI health by facilitating collaborative efforts and partnerships among
private and public health organizations and professionals, academic
institutions; |
|
|
|
Enhance AAPCHO’s capacity to increase awareness of policymakers and health
advocates of AAPI health issues; |
|
|
|
Increase the capacity of CHC members to deliver culturally-appropriate public
health services to underserved AAPIs; |
|
|
|
Increase the national availability and accessibility of health education,
promotion and prevention information for AAPIs; |
|
|
|
Create and maintain National Diabetes Education Program web portal for AAPIs. |
