DESCRIPTION OF CERTIFICATE OF CONFORMITY FORM

The form of the certificate of conformity is a sheet of paper format “A4” placed in the vertical position.

The form is made on a paper with the mass equal to 70 - 120 g/m² with contrast watermarks (rhombus) for reliable visual control. The paper should have no luminescence (seen luminescence) under exposure to ultra-violet radiation,

contain not less than two kinds of the fibers controllable in the seen or other areas of the spectrum.

The form of the certificate of conformity is framed by the stylized framework with

inclusion on its perimeter of the multiply-repeated microtext "Certification System in Communication," divided with the mark "*".

The internal part of the framework contains the superposition of two background grids with a variable step, one of which is iris.

III.          Description Of Certificate Of Conformity Positions

In certificate of conformity positions there are specified the following data:

1) Name of the certification system;

2) Name of the document;

3) Registration number of the certificate of conformity in the registry of certificates of conformity of the certification system in communications;

4) Period of the certificate of conformity validity. The dates are written as follows: Date (two digits)

Month (written)

Year (four digits);

5) Short name of the certification body, its address;

6) Name of the certified communication device, version of software (at presence), and the requisites of specifications;

7) Name of the manufacturer of the certified communication device, its address;

8) Name of the enterprise (factory) on which the certified communication device has been made, its address;

9) Requirements to conformity under which the communication device has been certified;

10) Application conditions for the communication device;

11) Name of the holder of the certificate of conformity, its address;

12) Number of sheets in the Appendix (digits);

13) Head/deputy head of the certification body;

14) Signature of head/deputy head of the certification body;

15) Name head/deputy head of the certification body;

16) Place for a seal;

17) Registration number of the form of the certificate of conformity.

On the certificate of conformity there is no numbering of positions.

At the presence of the appendix, each sheet is made out on the form of the certificate of conformity, subscribed by head/deputy head of the certification body and certified with a seal.

The data, which have no place in certificate of conformity positions 6), 7), and 8), are specified in the appendix.

Numbers of positions of the appendix are filled, as it is specified in the description of certificate of conformity positions.
