Type of Information Collection Request: New Collection, Regular; Title of Information Collection: Adolescent Family Life Pregnancy Prevention Core Evaluation; Form/OMB No.: OS-0990-New; Use: The Office of Adolescent Pregnancy Programs (OAPP) has developed core data collection tools to assist programs that have received Adolescent Family Life (AFL) demonstration grants with evaluating the programs and services provided as a part of their grant activies. These would be available to support both its prevention and care demonstration projects. The data collection tool for AFL prevention grantees will provide information on grantee progress in three areas: Reducing sexual risk behaviors, strengthening parents and families, and strengthening school and community supports.
