IAORDER 08/20/2008 On August 20, 2008, an Immediately Effective Order was issued to an individual prohibiting him from involvement in NRC-licensed activities for a period of one year from the date the Order was issued. The Order was issued based on his engagement in deliberate misconduct which caused his employer, Alaska Industrial X-Ray, Inc. to be in violation of 10 CFR 34.41(a) and for providing the NRC with information that he knew was inaccurate or incomplete.
