PERS provides all online forms and publications in PDF format. They can be opened using Adobe Reader© software. Please ensure you have the most recent version of Adobe Reader©.

You can fill out these forms on your computer. Simply use the "tab" key to move from one field to the next. You can also use your mouse to move from field to field. To do this, position the cursor in the field and click with your mouse.
Print out the form before you fill it in online to make sure your printer has the capability of printing the form correctly.
Each form has its own instructions. Make sure you read these completely, follow all instructions, and sign and date where indicated. Incorrectly completed or unsigned forms will cause a delay in processing your information.
All forms may be mailed to OSGP; some forms may be faxed.
