No. SSA does not have a form for direct deposit of benefits. The easiest way for beneficiaries to start or change direct deposit is to either use our online application at, call Social Security at 1-800-772-1213 or contact their financial institution.

SSA can also use Treasury Department form, SF-1199a to start direct deposit, available in Portable Document Format (PDF) at  The completed form can be mailed to your local Social Security office which you can locate at
