This national map of age-adjusted and spatially smoothed heart disease death rates at the county level, for ages 35 and older, indicates that during the years 1999-2003 the counties with the highest heart disease death rates (i.e. in the top quintile) were located primarily in southern Appalachian states, states along the lower Mississippi River Valley, parts of Alabama and Georgia, and much of Oklahoma. Counties with the lowest heart disease death rates ( i.e. lowest quintile) are located primarily in the western half of the country and parts of the northern plains.

This national map of age-adjusted and spatially smoothed stroke death rates at the county level, for ages 35 and older, indicates that during the years 1999-2003 the counties with the highest stroke death rates (i.e. in the top quintile) were located primarily in states along the southeastern seaboard the lower Mississippi River Valley. Counties with the lowest stroke death rates (i.e. lowest quintile) are located primarily in parts of the South West region, parts of the Great Plains, parts of the northeastern states and much of Florida.
