Counties and cities not sampled or with sample size <100 (Alabama's Jefferson County; Denver, Colorado; Louisiana's Orleans Parish; Missouri's St. Louis County and City; Nevada's Clark County; Ohio's Franklin County; and Tennessee's Davidson County) are not reported on in this table.
