Non-Attorney Positions - Security Forms
Welcome to the U.S. Attorney’s Office, Southern District of New York. If you have received and accepted a job offer with this Office, you will be required to complete and return a security package. You have been registered in the on-line security system called E-Qip, and have been sent an e-mail from the Human Resources Office with the E-Qip web site address and 3 “golden questions” to access your security package. You should retain that e-mail and refer to it when accessing the E-Qip web site.
Please follow all instructions carefully!
- OF-306 - Declaration of Federal Employment
-DOJ-555 - Disclosure and Authorization Pertaining to Consumer Reports Pursuant to the Fair Credit Reporting Act
- -Selective Service - You can call the Selective Service office at (847) 688-6888 if you cannot recall your registration number.
- -Dual Citizenship & Loyalty Statement - If applicable, fill out both. Applicant certifies s/he is a dual citizen with the U.S. & another country. Submit this form and submit one copy of the following, in addition:
- Letter
of explanation stating why the applicant maintains dual citizenship with the
U.S. & another country.
- -Foreign Born Relatives or Associates Statement - You must include any relative or associate who was born in a foreign country even if they currently hold U.S. citizenship.
- -OFI-36 - Additional Information
Complete this form if you have immediate family members who are foreign born, a foreign born spouse, and/or a cohabitant (regardless of citizenship).
This form is only applicable under the following conditions:
1) Immediate family members are resident aliens or U.S. citizens other than by birth
2) Current spouse is foreign born
3)
Co-inhabitant(s) and any other persons to whom applicant is bound by affection
or obligation who may be
subject to duress by a foreign power
If the OFI-36 is applicable, also complete the Foreign National Relatives or Associates form describing the nature, frequency, and degree of your contact. The form can be found in the above listed Dual Citzenship information bullet.
- SF-87A Fingerprint Cards
Fingerprint cards will be mailed to you upon request and you can be fingerprinted
at any local police station. Or, you can make an appointment to return your
completed forms and be fingerprinted in the Human Resources Security Office.
