    The Department of Defense has submitted to OMB for clearance, the 
following proposal for collection of information under the provisions 
of the Paperwork Reduction Act (44 U.S.C. Chapter 35).
    Title, Associated Form, and OMB Number: Application Forms Booklet, 
Naval Reserve Officers Training Corps Scholarship Program; CNET 1533/
74, 1533/91, 1533/87, 1533/92, 1533/88, 1533/93, 1533/89; OMB Number 
0703-0026.
    Type of Request: Reinstatement.
    Number of Respondents: 14,000.
    Responses per Respondent: 1.
    Annual Responses: 14,000.
    Average Burden per Response: 4 hours.
    Annual Burden Hours: 56,000.
    Needs and Uses: This collection of information is used to make a 
determination of an applicant's academic and/or leadership potential 
and eligibility for a Naval Reserve Officers Training Corps (ROTC) 
scholarship. The information is used to select the best qualified 
candidates. Information is provided by the applicant and by high school 
administrators, teachers, and other adults well acquainted with the 
applicant.
    Affected Public: Individuals or Households.
    Frequency: On Occasion.
    Respondent's Obligation: Required to Obtain or Retain Benefits.
    OMB Desk Officer: Mr. Edward C. Springer.
    Written comments and recommendations on the proposed information
