Office of Labor-Management Standards (OLMS) Which Labor Organization Annual Report Form Do I Use? Were your labor organization's total annual receipts less than $10,000? Q. Did your labor organization's fiscal year begin July 1, 2004 or later and were the total annual receipts $250,000 or more? A. You must file the Revised Form LM-2 which you can download here. Q. Did your labor organization's fiscal year begin prior to July 1, 2004 and were the annual receipts $200,000 or more? A. You can file Form LM-2 using the Informed Filler version CD which you can order by calling 202-693-0123 or by sending an email to olms-public@dol.gov or you can complete the Revised Form LM-2 which you can download here. Q. Did your labor organization's fiscal year begin July 1, 2004 or later and were the total annual receipts $10,000 or more but less than $250,000? A. You can file Form LM-3 in the Adobe Reader 6-8 compatible versions which you can download here effective December 30, 2005. (However, if your labor organization is in trusteeship you must file Form LM-2.) Q. Did your labor organization's fiscal year begin prior to July 1, 2004 and were your total annual receipts $10,000 or more but less than $200,000? A. You can file Form LM-3 in the Adobe Reader 6-8 compatible versions which you can download here effective December 30, 2005. (However, if your labor organization is in trusteeship you must file Form LM-2.) Q. Were your labor organization's total annual receipts less than $10,000? A. You can file Form LM-4 in the Adobe Reader 6-8 compatible versions which you can download here effective December 30, 2005.
