The Telecommunications Equipment Purchase Program (TEPP) is to
help people with disabilities buy specialized equipment they need
in order to use basic telephone services. The TEPP is one of several
different programs paid for by the Wisconsin Universal Service Fund
(USF) established by the Public Service Commission. Money collected
from Wisconsin telephone service providers goes into the USF.

To apply for a TEPP voucher on-line, use our new on-line application
below. If you are unable to use the on-line application, you can
print the application form provided in pdf format and mail to the
address on the form.

Important Notice:
It generally takes 3 to 4 weeks to receive your voucher. It will take
longer to receive your voucher if you apply for TAP to cover
the copayor you choose not to provide your social security number. Applications
are processed in the order in which they are received and vouchers
are issued on a first come, first served basis. We encourage you to use the on-line application instead
of mailing or faxing applications as this will result in a much
quicker turn around time for voucher processing. |

Voucher Status |
When you open the "Voucher Application, On-line
TEPP Application", you can also check on the last time you used
TEPP funds. Fill out the status with your name and phone number(the
same as when you last applied) and the database will tell you
when you last completed a TEPP voucher or the status of your
current application. You are eligible to receive another voucher
three years from the voucher completed date. If the three years
have elapsed, please click on the "Click here for On-line
TEPP Application Form" and proceed to fill in all the requested
information. If the database does not give you any status information,
please contact the TEPP administrator at TEPP@Wipfli.com |
