Biographical Note

U.S. heart surgeon and medical investigator Adrian Kantrowitz was born on October 4, 1918 in New York City, to general practitioner Bernard Abraham and his wife Rose. Adrian Kantrowitz was responsible for pioneering developments in circulatory assist devices, artificial organs, medical electronics, heart transplantation, and research motion pictures.

His interest in medical research began as a child through kitchen experiments conducted with his older brother Arthur, who eventually became a physicist and cofounder of Avco Research Laboratories. Adrian received his bachelor's degree in mathematics from New York University in 1940 before attending the Long Island College of Medicine, now known as the State University of New York (SUNY) Downstate Medical School. Due to the growing need for doctors during World War II, he underwent an accelerated program and was awarded his medical degree in 1943.

A nine month internship at the Jewish Hospital of Brooklyn fostered Kantrowitz's initial career interest in neurosurgery. While there, he proposed a new type of clamp for performing craniotomies during intracranial surgery that became the subject of his first published paper, "A Method of Holding Galea Hemostats in Craniotomies," in 1944. World War II halted Kantrowitz's research work for two years while he served as a battalion surgeon in the U.S. Army Medical Corps. After being discharged in 1946 with the rank of major, Kantrowitz switched his specialty to cardiac surgery because of the lack of available training positions in neurosurgery. He would spend the rest of his career devoted to cardiovascular research, typically working eighteen-hour days six days a week.

From 1948 to 1955, Kantrowitz held various positions at the Montefiore Hospital in the Bronx. Through his experimental work with animals, Kantrowitz began developing an artificial left heart (a device he would continue refining for many years), a rudimentary oxygen generator for a heart-lung device, and a treatment for coronary artery disease that involved surgically rearranging blood vessels. This practice of testing experimental surgeries on animals (particularly dogs) before human beings continued throughout his career. In 1954 with the help of Alan Lerrick, a colleague at Montefiore Hospital's Surgical Research Laboratory, he invented a plastic heart valve that served as an extra heart pumping blood into anemic organs. During his time at Montefiore, Kantrowitz entered a two-year fellowship under distinguished cardiovascular physiologist Carl Wiggers at the Western Reserve University School of Medicine in Cleveland.

During his time at Montefiore Hospital, Kantrowitz filmed the first of many pioneering research motion pictures. His film offered a color close-up look inside a living dog's heart while the organ was still inside its chest. The artificial heart designed by Kantrowitz to keep the dog's heart free of blood while supplying blood to the rest of the body also provided a clear view of the living organ in action. On October 16, 1951, he screened the world's first motion picture of the inside of a living heart before 350 physicians at the annual Graduate Fortnight of the New York Academy of Medicine.

Kantrowitz began a fifteen-year professorship at SUNY Downstate Medical Center in 1955 and established a lasting friendship with Chief of Surgery Clarence Dennis. That same year, Kantrowitz received an appointment to the full-time position of Director of Cardiovascular Surgery at the Maimonides Hospital (later known as Maimonides Medical Center), a small community hospital in Brooklyn. For the next fifteen years, Kantrowitz held a variety of positions at Maimonides and led a research team that devised a series of innovative electronic devices to compensate for various physiological malfunctions. These devices included the extremely sensitive, electronically controlled heart-lung machine (developed in 1958) that allowed him to execute a series of successful open heart surgeries in children. Kantrowitz also created a diaphragm booster heart with William M. P. McKinnon that functioned as a second heart, relieving the natural heart of 25% of its work. In 1960, Kantrowitz and Rene Khafif built a large electronic system to deliver electric shocks to anesthetized dog legs and produce walking motions, an important advancement in paralysis treatment. A mini radio transmitter emitting signals that caused paralyzed human bladders to empty was also developed around this time. Kantrowitz also collaborated with the Electronics Laboratory at General Electric to successfully manufacture an externally controlled, internal cardiac pacemaker to implant in human patients suffering from heart ailments such as Stokes-Adams disease.

Building upon past artificial left heart and booster heart experiments, Kantrowitz and collaborators such as his brother Arthur, Tetsuzo Akutsu, Franz Gradel, and Paul-Andre Chaptal worked throughout the 1960s on a mechanical auxiliary left ventricle to aid the failing heart. Their goal was to create an externally controlled, valveless, internal pump synchronized with the body's regular pumping cycle through the natural heart's electrical activity that bypassed the left ventricle. After numerous dog tests, Kantrowitz carried out the world's second permanent partial mechanical heart implantation in a human on February 4, 1966. Although the auxiliary ventricle worked, the patient died after twenty-four hours from advanced liver disease. Kantrowitz's second highly publicized attempt at implanting a partial mechanical heart in 63-year-old Louise Ceraso on May 18, 1966, also ended in disappointment when she died of a stroke thirteen days later. Kantrowitz later expressed the opinion that the difficulties faced by contemporary Michael E. DeBakey and himself suggested that effective mechanical heart implantation was still many years away.

The intra-aortic balloon pump created by Kantrowitz's team during this period proved to be a more successful heart assist device. Devised as a quick, non-surgical form of mechanical heart assistance, the balloon pump facilitated increasing blood flow and oxygen to the heart muscle until the ventricle could resume pumping on its own. A trial run on a female patient in 1967 significantly improved her health, and Kantrowitz subsequently used the balloon pump to treat patients suffering from myocardial infarction.

Kantrowitz was a major contender in the race to perform the world's first total human heart transplant. His team transplanted hearts in 411 dogs over a five year period in preparation for human trials. The surgery required a suitable donor and recipient at the same time, which occurred in late 1967. On December 6, 1967, three days after Christiaan Barnard's historic surgery in South Africa, Adrian Kantrowitz completed the United States' first and the world's second human heart transplant. A 2.5-week-old male infant suffering from severe tricuspid atresia received the heart of a two-day-old male infant dying of anencephalitis. The surgery was the first performed on an infant and lasted 2.5 hours; the recipient lived only 6.5 hours. After the death of his second patient (an adult) in January 1968, Kantrowitz returned to experimentation in the face of growing public criticism against cardiac transplants.

Ethical questions about the scope of Kantrowitz's research escalated existing tension with the administration at Maimonides Hospital. In 1970, Kantrowitz relocated his entire team of surgeons, researchers, biomedical engineers, and nurses to Sinai Hospital of Detroit, Michigan, where he was now an attending surgeon and Chairman of the Department of Surgery. He held several positions at Sinai Hospital over the next three decades and also served as Professor of Surgery at the Wayne State University School of Medicine. In this new work environment, Kantrowitz continued experimenting with heart transplants, the balloon pump, and partial mechanical hearts. In August 1971, he implanted an artificial heart booster (a more advanced version of the auxiliary left ventricle) into 63-year-old Haskell Shanks, who became the first partial mechanical heart patient to be sent home. He died three months after the surgery. Following this and other ultimately unsuccessful surgeries, Kantrowitz shifted his primary focus to partial mechanical devices that assisted rather than replaced the natural heart, considering artificial hearts and cardiac transplants too impractical for the foreseeable future.

Adrian Kantrowitz married Jean Rosensaft on November 25, 1948; she worked on the administrative staff of the surgical research laboratories at Maimonides Medical Center during his tenure there. In 1983, Dr. and Mrs. Kantrowitz cofounded L.VAD Technology, Inc., a Detroit-based research and development company specializing in cardiovascular devices. Dr. Kantrowitz and his wife currently are running the company as president and vice president respectively. The couple has two daughters, Niki Ellen and Lisa Robin, and a son, Allen Bernard.

Grant material, experiment data, patient medical records, correspondence, publications, conference material, publicity clippings, and audio-visual media (64 linear feet; 1944-2004) document Adrian Kantrowitz's professional career as a cardiovascular surgeon and investigator. Kantrowitz devoted his research to devising surgical procedures and mechanical assist devices to correct numerous physiological ailments, with a particular focus on aiding or replacing the failing heart. Six research project series (Series II-VII) document the surgical research conducted and mechanical devices developed by Kantrowitz and his colleagues at the Maimonides Hospital in Brooklyn and the Sinai Hospital of Detroit. They contain raw data and patient files from animal and human experiments, along with administrative records, correspondence, publications, and other material related to the development and promotion of their research. Records cover the following projects: Artificial Left Heart, Electronic Physiologic Aids, Mechanical Auxiliary Ventricle, Intra-Aortic Balloon Pump, Heart Transplant, and Other Research Projects. The largest group of records is devoted to Kantrowitz's attempts at heart transplantation, with special attention given to the first human heart transplant performed on an infant in 1967. Series I: Grants and Funding, 1950-1990 6.67 linear feet plus 4 oversize folders

The Grants and Funding series contains material related to the acquisition and renewal of funds to support the various research projects of Kantrowitz and his research team. There are three sub-series: Grants, Foundation Information, and Senate Testimony. The Grants sub-series documents the many grants that funded Kantrowitz's various circulatory studies, mechanical organ assist device projects, and experiments in implantation methods. The "Studies in Assisted Circulation" grant alone encompasses a variety of projects that include the dynamic aortic patch, U-shaped mechanical auxiliary ventricle, phase-shift balloon pump, and the dynamic prosthetic myocardium. This sub-series consists of applications, proposals, progress reports, drafts, correspondence, budget information, invoices, expense reports, floor plans, and invention statements. Grants are arranged chronologically by their initial start date, and folders are arranged chronologically within their respective grant titles.
Drafts and completed versions of grant applications and proposals are restricted until 75 years after date of submission.
The Foundation Information sub-series contains correspondence with and background information on private foundations. It is arranged alphabetically by foundation name, with foundations named after people filed under their surnames and summary binders which cover multiple foundations grouped at the end.
The three folders in the Senate Testimony sub-series contain evaluations on the adequacy of federal support for biomedical developments; they are arranged alphabetically by folder title.

Series II: Artificial Left Heart (ALH), 1958-1971 (Bulk 1965-1966) 1.19 linear feet plus 1 oversize folder

These materials document Kantrowitz's work on the "Studies Basic to Consideration of Artificial Heart Research" contract. Here, the term "artificial heart" refers to both the auxiliary left ventricle and the implantable artificial heart. The contract was a collaborative effort between Arthur Kantrowitz's colleagues at the Avco Corporation and Adrian Kantrowitz's team at Maimonides Hospital, which included his wife Jean, Tetsuzo Akutsu, Paul-Andre Chaptal, Georges Dureau, and Toru Okura. The series consists of correspondence, memoranda, administrative lists, background material, publications, time cards, and draft and master copies of the different sections of a task report which summarized the entire study. Materials are arranged alphabetically by folder title.

Series III: Electronic Physiologic Aids (EPA), 1954-1972 (Bulk 1961-1966) 3.52 linear feet plus 2 oversize folders

Materials in the EPA series describe several research projects, each concerned with developing devices or surgical methods which compensate for various heart, limb, and gastrointestinal ailments. The experimental, administrative, and publicity aspects of these different projects are represented to varying degrees in the following six sub-series: Pacemaker, Adynamic Paralytic Ileus Stimulation, Phrenic Nerve Stimulation, Bladder Stimulation, Coordinated Limb Movement, and Other. The Pacemaker and Bladder Stimulator projects are covered most extensively and required further subdivision. Specific material types throughout the EPA sub-series include correspondence, professional and layperson inquiries, patient lists, medical files, human and animal data sheets, publications, forms, printed matter, photographs, drafts, and memoranda. Folders within each sub-series are arranged alphabetically by folder title.

Series IV: Mechanical Auxiliary Ventricle (MAV), 1952-1989 (Bulk 1965-1967) 3.83 linear feet plus 3 oversize folders

This series documents the experiment data gathered in animal tests of the MAV, alternately referred to as the mechanical booster heart (MBH). The vast majority of the folders contain the raw data and handwritten notes recorded during the implantation of the U-shaped MAV model by Kantrowitz and such colleagues as Paul-Andre Chaptal, Franz Gradel, Toru Okura, Tetsuzo Akutsu, and Wladimir Schilt. All experiment logs, notes, ECG recordings, pathology reports, autopsies, and photographs are of animal experiments (primarily dogs). Sub-series within the MAV series are based on investigator or experiment series names and are in rough chronological order. Folders within these sub-series are arranged alphabetically by folder title or numerically by experiment number, although the Mechanical Booster Heart Tracings are arranged chronologically. An alphabetically arranged General sub-series at the end gathers together various MAV-related records, including random data, correspondence, and patent application material.

Series V: Intra-Aortic Balloon Pump (IABP), 1963-1989 (Bulk 1967-1977) 7.29 linear feet plus 24 oversize folders

The IABP series is comprised of four sub-series: Dog Studies, Human Studies, Instructional Training Program, and Cooperative Study. The last three sub-series are subdivided further. Materials in the first two sub-series comprise experimental data gathered during animal and human trials with the IABP. Dog Studies consists entirely of balloon pump readings that are arranged alphabetically by folder title; Human Studies consists of data logs, medical files, case reports, patient lists, ECG recordings, and wall charts as well as information on a study run by Mary Caiazza and Jaroslav P. Stulc. Most folders within these sub-series are arranged alphabetically by title. The Caiazza-Stulc study patient lists follow the order listed in the Table of Contents; the Case File notebooks which summarize numerous patients are arranged before the individual case files.
The third sub-series, Instructional Training Program, documents Kantrowitz's efforts to promote a wider acceptance of the IABP by training other healthcare professionals in its use. Particularly well documented is a one-time training workshop run by Sinai Hospital of Detroit in cooperation with the Ohio State University Perfusionist Training Program. Material types in this sub-series include correspondence, memoranda, evaluation and registration forms, background research, interviews, and printed matter. The Cooperative Study sub-series refers to a proposed effort by Maimonides Hospital and other institutions to construct IABP devices and deliver them to other medical facilities. Included are program proposals, reports, meeting minutes, correspondence, memoranda, manuals, and two audio reels. The subdivisions within each sub-series are arranged alphabetically by title; folders within these subdivisions are arranged chronologically by date range, then alphabetically by title.

Series VI: Heart Transplant (HT), 1961-2004 (Bulk 1966-1969) 8.13 linear feet plus 7 oversize folders

This series provides details on the Kantrowitz research team's experimentation with cardiac transplants in animals and human beings. It consists of surgery records, correspondence, professional and layperson inquiries, memoranda, reports, reprints, abstracts, consent forms, conference material, manuscript drafts, background research, and clippings. The series is divided into the following sub-series: Surgery Data, Administrative Files, Publications, Nancy Ross-Flanigan Files, BBC Documentary, and Michael Meyer Files. The first four sub-series are subdivided further. Materials in the first sub-series are arranged alphabetically by folder title or numerically by experiment numbers. Folders in the second, third, and fourth sub-series are arranged chronologically by date within their respective subdivisions. The final two sub-series are also arranged chronologically by folder date.
Surgery Data contains images of the infant transplantation surgery performed by Kantrowitz on December 6, 1967, as well as the raw data, handwritten notes, and ECG recordings from numerous dog and human infant trials performed by Yasunori Koga and Shigehisa Yagi. The second and third sub-series document internal developments and public discussions regarding animal and human heart transplants, such as the surgeries on the dogs Pirate and Kuru and the ethics of total heart transplants. The Nancy Ross-Flanigan Files contain the planning documents, research, and drafts kept by Ms. Flanigan to help Kantrowitz prepare "America's First Human Heart Transplant -- The Concept, the Planning, and the Furor," a 30th anniversary retrospective on the surgery. The fifth sub-series consists of correspondence regarding a BBC documentary on the history of transplant surgery, and the final sub-series is a group of writings and articles on Kantrowitz's first human heart transplant.

Series VII: Other Research Projects, 1955-1985 2.81 linear feet plus 12 oversize folders

This series brings together files from five less heavily documented projects. Documented here are Cardiovascular (CV) Surgery, Aortic Valve Transplant, Assisted Circulation Engineering, Prosthetic Myocardium, and Chronic Heart Failure (CHF). Experiment notebooks, log sheets, patient records and readings, photographs, correspondence, blueprints, expense reports, invoices, and equipment proposals can be found throughout this series. The CV Surgery sub-series contains notebooks kept by Arthur Cohen and Fred Heilman that cover several experiment series involving dogs, and the Assisted Circulation Engineering project contains planning and research records regarding the construction of a new heart surgery study area at Maimonides Hospital. Folders are arranged alphabetically by folder title within each sub-series.
Portions of the series are restricted by HMD's Access to Health Information of Individuals policy (PHI).

Series VIII: Publicity, 1949-1989 0.92 linear feet plus 12 oversize folders

Publicity series materials include newspaper clippings, magazine articles, correspondence, printed matter, and photographs which report on Kantrowitz's activities and notable events in the thoracic field and general medicine. All acidic newspaper clippings were photocopied onto acid free paper; the originals were discarded and the photocopies were retained. There are two sub-series: the Research and Professional Activities sub-series and the General Medical Subject Files sub-series. The primary focus of the Research and Professional Activities sub-series is Kantrowitz's research, promotions, conference attendance, and speaking engagements, but it also documents accomplishments of contemporary surgeons such as Michael E. DeBakey and Christiaan Barnard. The General Medical Subject Files sub-series covers a broad range of trends and achievements both in Kantrowitz's area of expertise and other medical fields. Items within each sub-series are arranged chronologically by date.

Series IX: Clinical Films, 1950-[ca.2004] 18.25 linear feet

The Clinical Films series contains 8 and 16 mm open reel films, the majority of which were filmed without sound. Included are fragments, duplicate footage, and completed films that document experimental operations performed by Kantrowitz and his associates. Kantrowitz screened completed films such as "Experimental artificial left heart for exposure of the mitral valve," "Repair of ruptured ventricular septum," and "Long term survival after canine heart allotransplantation" during presentations at various professional events and continuing education courses. Both animal and human surgeries are represented, with puppy transplants (including that of the dog Pirate) making up a substantial portion of the series. Clinical Films is sub-divided by surgery categories: Artificial Left Heart/Mitral Valve, Cardiovascular Surgery, Electronic Physiologic Aids, Haskell Shanks, Heart Transplant, Heart Transplant (Puppy), Intra-Aortic Balloon Pump, Mechanical Auxiliary Ventricle, Orsi, Patch Booster Dog, Prosthetic Myocardium, and Other. Films within each sub-series are arranged numerically by the line numbers included in the film titles. The films have been rehoused in archival canisters, assigned item numbers, and stored separately from the rest of the collection. Nineteen films covering a variety of surgeries were digitized onto three Panasonic-DV cassettes. These cassettes are located in the folder "LVAD digitized films, tapes 1-3" at the end of the series inventory.

Among the subjects covered are: peripheral arterial blood flow, urinary bladder stimulation, heart transplantation, the implantable cardiac pacemaker, a percutaneous energy transmission system, and various forms of the mechanical auxiliary ventricle. Writings is arranged in two sub-series: Journal Articles and Draft Material. The first sub-series includes a numbered bibliography of selected Kantrowitz writings, which are in rough chronological order. All of the published journal articles, reprints, and reports from this bibliography are arranged by their bibliography number. The second sub-series consists of miscellaneous rough drafts and illustrations from published works; it is arranged alphabetically by folder title.

This series contains speeches, lists of speeches, transcripts, abstracts, drafts, programs, correspondence, clippings, and printed matter separated from Series XII: Conferences, Meetings, and Events. The speeches in these folders were delivered by Kantrowitz and others at several professional events in the 1960s. Speech subjects include the future of cardiology and advancements in cardiac support devices. Researchers should note that the series is not a comprehensive gathering of Kantrowitz's talks. Mention is made in Series XII folders of other presentations given by Kantrowitz, but those folders do not contain the actual speeches. The material is arranged chronologically by event date.

Series XII: Conferences, Meetings, and Events, 1958-1985 (Bulk 1966-1973) 4.08 linear feet plus 1 oversize folder

This series provides information about the various conferences, workshops, meetings, dinners, award ceremonies, and other professional events Kantrowitz attended or was invited to over a thirty-year period. These are regional, national, and international events devoted to topics from cardiovascular surgery, thoracic surgery, and medical and biological engineering. One folder of note covers an exchange of surgical apparatus and information that followed Kantrowitz's visit to the USSR in the late 1950s. Materials types found in Series XII include programs, correspondence, travel kits, manuscripts, drafts, membership applications, printed matter, name tags, and expense reports. The files are arranged chronologically by event date. Folders with lists of multiple events are filed at the beginning of the year the first event takes place (e.g. the folder "National meetings (July 1, 1970 to December 31, 1971)" is filed at the beginning of 1970).
