Subject: Weldments on Opposed Piston and Coltec-Pie(stick Emergency

Stand-By Diesel Gen-Set Lube-Oil and Jacket Water Piping Systems



This is a supplement to the Interim Letter dated September 30, 1997, and

to previous supplements 97-00201, 97-002-02, and 97-002-003 dated

December 18, 1997, March 3, 1998, and April 29, 1998 respectively.  It is

intended to inform the U.S. Nuclear Regulatory Commission, and the

affected nuclear utilities on the progress of the root cause

investigation, and corrective/preventative actions associated with the

Coltec-FMED Part 21 File No. 97-002.



Supplement 97-002-03 previously stated that the initial conclusion is the

weld joints were of adequate strength for the application based on

tensile tests.  In addition, Supplement 97-002-03 stated that the

metallurgical examination of the weld joints and review of the customer

contract requirements would be completed at the end of May 1998.

However, due to limited engineering resources, only the review of

customer contract requirements and metallurgical examination of the weld

joints have been completed at this time.  The engineering analysis of the

metallurgical examination is in progress, and will be completed by August

14, 1998.  At the completion of this item, the final notification will be

submitted.



The completed macro and microstructure examination included removing

three samples from each weld.  Photomacrographs were taken to depict the

transverse view of the weld at each location.  Photomicrographs were

taken to depict the absence of crack propagation at the weld root and to

exhibit general structure.  The microstructure samples served to

determine the depth of weld penetration.  The results of the examination

will be used by engineering for analysis.  In addition, review of

customer contract requirements has been completed.  All customer contract

requirements were satisfied.  However, the requirements of the weld

procedures used by Coltec were not satisfied.  Immediate action taken

included correcting any in process welds and briefing weld personnel on

proper weld joint design.  Actions to preclude recurrence includes formal

training for all weld personnel on weld joint design, designation, and

interpretation, which is being tracked by Corrective Action Request 1750.



Previously submitted with Supplement 97-002-03 was a copy of Appendix E-1

describing the results of the tensile test.  Appendix E-2 will contain

the results of the metallurgical examination and photographs of the weld

joints when completed.  Appendix C exhibits photos of the sections of the

section of the piping as received.  Appendix D exhibits photos of the

test section after completion of the burst testing.  These three

appendices (C, D, and E-2) are not included in this supplement.  They

will be included the final notification.
