INSTRUCTIONS/NOTES:

  1. Fill out one form for each leased NLM database for which there is a product/service resulting from your use of the data. Do not submit a report for internal usage of NLM data for research purposes.
  2. Reporting period is NLM's Fiscal Year Oct. 1, 2007 - Sept. 30, 2008. If you cannot provide statistics on this 12-month period, let us know the period of time on which your data are based. Report is due December 5, 2008.
  3. Licensees may propose an alternative method of calculating connect hours. An alternative that NLM finds acceptable is to divide the total number of characters transmitted by the licensee by the average number of characters transmitted per hour to arrive at the equivalent number of connect hours. Please note on the form if you use this alternative method.
  4. Licensees may redistribute data to others per the NLM license agreement. It is the licensee's responsibility to inform those recipients of copyright, quality assurance, and reporting requirements. Usage of redistributed data should be included in the aggregate in your report to NLM.
  5. NLM has the responsibility to report usage of its data to the U.S. Congress.
