    Cruise Ship's Corrective Action Statement

Item No.: 08
Site: Other-Deck 4 Chemical Locker
Violation: It could not be determined a backflow prevention device was installed on the chemical dispenser.
Recommendation: The vessel shall provide a comprehensive cross- connection control program that provides safe connections to the potable water system through air gaps or appropriate backflow devices at any connection between potable and non-potable water systems.

Site: Potable Water-Cross-connection Control Program
Violation: The cross-connection control program did not include the chemical dispensers, the spa showers or the air gaps. This was corrected.
Recommendation: Ensure that the cross-connection control program includes at minimum: a complete listing of cross-connections and the backflow prevention method/device for each, so that there is a match to the plumbing system component and location, and an inspection frequency. Ensure that air-gaps are included in the listing.

Site: Swimming Pool-Crew Pool
Violation: The shepherd's hook was not located in a prominent location. This was corrected.
Recommendation: Ensure that an easy-access shepherd's hook and approved floatation device (with a length of line 1/2 the pool width or 16 m (50 feet)) is provided at a prominent location near each public swimming pool.

Violation: There was a box of bottles on the deck.
Recommendation: Ensure that food is protected from contamination by storing the food: (1) In a clean, dry location; (2) Where it is not exposed to splash, dust, or other contamination; and (3) At least 15 centimeters (6 inches) above the deck.

Violation: There was a wiping cloth on the edge of the sanitizing bucket.
Recommendation: Ensure that wet wiping cloths are stored in a chemical sanitizer.

Violation: There were two serrated knives stored in a cabinet with miscellaneous items (i.e. menus and paper goods).
Recommendation: Ensure that cleaned equipment and utensils are stored: (1) In a clean, dry location and (2) Where they are not exposed to splash, dust, or other contamination.

Violation: There was ice in the handwash sink.
Recommendation: Ensure that handwashing facilities are used for no other purpose.

Violation: There was food debris in the handwash station waste receptacle adjacent to the combination oven.
Recommendation: Ensure that handwashing facilities are used for no other purpose.

Violation: The temperature of the hot water at the handwash station was measured at 83° F. This was corrected.
Recommendation: Ensure that handwashing sinks are equipped to provide water at a temperature of at least 43°C (110°F) through a mixing valve or combination faucet.

Site: Other-Da Vinci Dining Room
Violation: There was food and other debris on the deck at waiter stations 77 and 79 in the gap between the end of the water stations and the columns.
Recommendation: Ensure that decks, bulkheads, deckheads, and attached equipment in food preparation, warewashing, pantries, and storage areas, are cleaned as often as necessary.

Site: Other-Horizon Court
Violation: In the pool area, deck cabinets were being used as soiled waiter stations. There was no coving around the cabinet and the deck was constructed of an absorbent material.
Recommendation: Ensure that decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas are constructed and maintained for easy cleaning.

Violation: There was water dripping from the deckhead to the deck on the clean side of the potwash machine. No clean items were impacted.
Recommendation: Ensure that ventilation hood systems and devices are sufficient in number and capacity to prevent grease or condensation from collecting on bulkheads and deckheads.
