Site: Medical-Clinical Specimen Containers
Violation: The specimen collection containers for the bacteria agents expired in July, 2008.
Recommendation: Ensure that at least 10 clinical specimen collection containers for both viral and bacteriological agents (10 each), as well as the proper shipping containers and labels for same, are maintained by the medical staff.

Site: Potable Water-Potable Water Distribution Records
Violation: On 29 and 30 March 2008, the distribution charts for the deck 8 chart recorder was at the top of the chart and appeared to be in excess of 5 ppm for several hours with no explanatory notes on the chart or in an engineering logbook.
Recommendation: Ensure that records from the halogen analyzer-chart recorder verify the free residual halogen between 0.2 mg/L (ppm) and 5.0 mg/L (ppm) in the water distribution system for at least 16 hours in each 24-hour period since the last inspection of the vessel.

According to the staff, there was an issue with the insulation around the pipes in the deckhead.
Recommendation: Ensure that a plumbing system in a food area is maintained in good repair.
