  The Benefit Eligibility Screening Tool (BEST) asks for certain dates such as your date of birth, and, if you are or were married, your spouse's date of birth and the date of your marriage. (If you are unsure of the exact dates, you can enter an estimated date.) It will also ask about the amount of income you receive and the amount of your assets.
