DESTINATION CODES AND ADDRESSES
FOR CLAIM FILE SHIPMENT
IN
FAVORABLE APPEAL COUNCIL (AC) DECISION CASES
All favorable AC decision Title II disability claim files go to Office of Disability Operations (ODO) via the Office of Hearings and Appeals mailroom shuttle, UNLESS the claimant is age 55 years old or older. The location code for ODO is 0PC7.
All favorable AC decision Title II non-disability claims files and all favorable AC decision Title II disability claim files for claimants age 55 years old or older go to the Program Service Center (PSC). The appropriate PSC is determined by the first 3 digits of the claimant's Social Security number, as indicated on the list of PSC's addresses and location codes (below).
All favorable AC decision Title XVI claim files go to the servicing Social Security Office. Consult DOORS for the appropriate office, address, and location code.
If the claim file is sent via express mail, the street address of the destination must be used, rather than the P.O. Box. Consult DOORS for the appropriate street address.
All favorable AC decision transmittal documents (HA-505) must contain, in capitalized, bold, highlighted letters, the following remark: EFFECTUATION NECESSARY. The form to create this document is located under the MISC tab of the Office of Appellate Operations' (OAO's) Document Generation System (DGS) at “MISC 505 Transmittal.”
All AC Decisions must contain a completed AC Decision Coding Sheet. The form to create this document is located under the DEC tab of OAO's DGS at “Flag for AC Decision Codes.” See I-3-8-91 for an example of the AC Decision Coding Sheet.
