Background: From October 2000 through June 2008, the Iowa Department of Public Health was funded as an enhanced research project.

Lifestyle Interventions: Iowa has revised its Iowa Care for Yourself, based on the Dietary Approaches to Stop Hypertension (DASH) diet. Revised Iowa Care for Yourself is a six–session DVD-based format recorded by Cooperative Extension health educators. Each session presents a healthy snack, topic presentation, brief physical activity, skill building activity, behavior change and goal setting activities, and evaluation.

Screening: Risk factor screening includes blood pressure, glucose, and lipids testing, and assessment of weight, medical history, tobacco use, diet, and physical activity status.

Sites: Twenty sites including health departments, hospitals, visiting nurse associations, home health services, and women’s service agencies.

Key Partners: University of Iowa at Iowa City and Iowa State University Extension Services at Ames.
