Preventive Health and Health Services Block Grant

The health needs of communities are diverse, complex, and
constantly changing. The Preventive Health and Health Services (PHHS) Block
Grant gives grantees the flexibility to prioritize the use of funds to fill
funding gaps in programs that deal with leading causes of death and
disability, as well as the ability to respond rapidly to emerging health
issues including outbreaks of food borne infections and water borne
diseases. The PHHSBG has provided 50 states, the District of Columbia, 2
American Indian tribes, and 8 U.S. territories with funding to address their
own unique public health needs and challenges in innovative and locally
defined ways.

action. The state, territory, and tribe overviews and success stories found
in this publication illustrate the wide range of activities for which PHHS
Block Grant funds are used to address public health needs, improve community
health, save lives, and leverage or secure health care dollars.

Public Reporting Burden Statement:

30 hours/minutes per response for the application and 40 hours/minutes per
response for the annual report, including the time for reviewing
instructions, searching existing data sources, gathering and maintaining the
data needed, and completing and reviewing the collected information. An
