Locating and ordering DHS forms

The DHS Forms server contains many DHS forms and publications in Microsoft Word, Excel and Adobe Acrobat Reader® formats.

 

A number of forms are available in languages other than English, as well as in large print.

 

How to locate a specific form

the DHS Forms/Pubs Search page offers many options for finding current and past DHS forms. When using the search page:

  • In the form number (Form #) field, use only numbers. Do not include any letters in this field.
  • If you don’t have a form number, you can use the keyword search to help locate it. Enter one or two words from the name of the form, such as food stamps or child care.
  • You can narrow your search by entering information into more than one field, or using more words in the 'title contains' field. Enter less information to find more forms.

Requesting revisions or a new form

DHS employees who would like to have an existing form revised, or a new one created, should submit their request to the DHS Office of Document Management (ODM), using the DHS 1001 Project Request Form. ODM will evaluate the request and contact the customer to begin production.

 

Ordering forms

DHS staff can order forms and publications in quantity using the Forms Branch Ordering System (FBOS). There should be a designated person in each DHS office who is responsible for ordering forms through this system.
