Alternate formats
DHS is committed to providing Oregonians with its forms and publications in appropriate alternate formats. This can include Braille, large print, audio tape, computer disk (in ASCII format) or oral presentation.
To obtain alternate formats:
- Check the DHS Forms/Pubs Search page. Some DHS forms and publications are available in large print from the DHS forms server.
- DHS clients should request alternate format materials from their DHS worker.
Translations
Currently, we translate forms and publications for clients into Spanish, Russian and Vietnamese. Certain forms are also available in Bosnian, Cambodian, Hmong, Laotian, Mien and Romanian.
When requesting a form or publication in an alternate format or a language other than English be sure to include the following information:
- The form or publication number or name
- Type of alternate format or language requested
- Quantity
- Date needed
- Name of person requesting the material
- Mailing address
- Phone number
- DHS office (if client or staff member)
