On April 7, 2004, a Notice of Violation was issued for a violation
associated with a White SDP finding involving the use of unapproved operator
manual actions for achieving and maintaining hot shutdown conditions in
the event of a fire in particular fire zones. The violation cited the licensee’s
failure to comply with fire protection regulations in 10 CFR Part 50, Appendix
R, Section III.G.2 requiring that cables and equipment of redundant trains
of systems necessary to achieve and maintain hot shutdown conditions remain
free of fire damage. |

III violation based on (1) inadequate design control over changes made to
the pump bearing housing material and the viscosity of the bearing lubricating
oil; and (2) the above changes resulting in the pumps being inoperable from
January 28 to February 5, 2000, when service water cooling temperatures
were below 42 degrees F. |
